How to file a claim under the BLET MetLife Short Term Disability insurance program

Call Cornerstone Benefits Management at 224-848-4941 to start a new claim, then continue as below:

  • Tell the Claims Advocate that you want to file a new claim under the “BLET Short Term Disability insurance plan”;
  • Provide the information requested from the Claims Advocate, including contact information for your attending physician(s);
  • During the call, the Claims Advocate should provide you with a 12-digit claim number, which you should keep available for future reference, a MetLife Case Manager will be assigned to your claim and
  • The Claims Advocate will inform you that MetLife is going to provide you with a medical release form. Complete and sign the medical release form provided by MetLife, and return the form to MetLife and send a second copy to your doctor per instructions by MetLife.
