Important notice regarding Form 1095-B for those covered by the GA-23000 Plan

INDEPENDENCE, Ohio, January 17 Due to a change in IRS requirements, most of those covered by The Railroad Employees National Health and Welfare Plan (GA-23000) members will no longer automatically receive a paper copy of the Form 1095-B. Members living in states that still require reporting of health coverage (e.g. New Jersey) will still automatically receive a paper copy of the Form 1095-B for state filing tax purposes.

Any GA-23000 Plan member that would like a copy of the Form 1095-B may request a copy by doing any of the following:

1. Call Railroad Enrollment Services at 1-800-753-2692.

2. E-mail a request to: railroad_enrollment_services@uhc.com Request must include your name, policy number, employee ID, employer name and current mailing address to send paper copy of Form 1095-B.

3. Mail a written request to: Railroad Enrollment Services, P.O. Box 30775, Salt Lake City, UT, 84130-0775). Please note your request must include your name, policy number, employee ID, employer name and current mailing address to send paper copy of Form 1095-B.

A Form 1095-B will be mailed to the address provided within 30 days of the date the request is received. If you have any questions about your Form 1095-B, contact Railroad Enrollment Services at 1-800-753-2692
