To All 2012 SWCM Scholarship Applicants
- The application is interactive and should be completed on-line and printed. Otherwise, please type application or print with black ink. Please do not fold, staple, or write on the back of the application.
- If the applicant is a high school senior, an official high school final grade transcript must be enclosed with the application.
- If the applicant is a college student, an official current college grade transcript must be enclosed with the application.
- All sections of the application must be completed. Incomplete applications will not be considered for a scholarship.
- If the applicant is married, spouse's name, social security number and occupation must be furnished.
- The applicant must take a minimum of twelve (12) credit hours (or equivalent) to be eligible.
- The application must be signed by the parent, the Division Secretary-Treasurer and the applicant.
- A photograph of yourself, suitable for publication, should be enclosed with the application.
- Return the application to the name and address listed on page four of the application.
- Applications must be postmarked no later than August 1, 2012. Applications will not be accepted after that date.
- Prior to the distribution of funds, the scholarship committee reserves the right to review scholarships for erroneous information and to rescind scholarships.
- If awarded a scholarship, the recipient must provide a letter of documentation from the university or college where enrolled, confirming that the recipient is a full time student. Documentation must also list what other grants, scholarships, aid or assistance the recipient will receive.
- Scholarships will be awarded at the Southwestern Convetion Meeting. Recipients of scholarships will be notified by US Mail after the meeting concludes on September 13, 2012. A list of the recipients will also be posted on the SWCM website found at http://bletswcm.org/index2.html. Inquiries can also be made by calling Norman Baker at 501-231-1927.
