To All 2009 SWCM Scholarship Applicants

  1. The application is interactive and should be completed on-line and printed. Otherwise, please type application or print with black ink. Please do not fold, staple, or write on the back of the application.
  2. If the applicant is a high school senior, an official high school final grade transcript must be enclosed with the application.
  3. If the applicant is a college student, an official current college grade transcript must be enclosed with the application.
  4. All sections of the application must be completed. Incomplete applications will not be considered for a scholarship.
  5. If the applicant is married, spouse's name, social security number and occupation must be furnished.
  6. The applicant must take a minimum of twelve (12) credit hours (or equivalent) to be eligible.
  7. The application must be signed by the parent, the Division Secretary-Treasurer and the applicant.
  8. A photograph of yourself, suitable for publication, should be enclosed with the application.
  9. Return the application to the name and address listed on page four of the application.
  10. Applications must be postmarked no later than August 1, 2009. Applications will not be accepted after that date.
  11. Prior to the distribution of funds, the scholarship committee reserves the right to review scholarships for erroneous information and to rescind scholarships.
  12. If awarded a scholarship, the recipient must provide a letter of documentation from the university or college where enrolled, confirming that the recipient is a full time student. Documentation must also list what other grants, scholarships, aid or assistance the recipient will receive.
  13. Scholarships will be awarded at the Southwestern Convention Meeting. Recipients of scholarships will be notified by U.S. Mail after the meeting concludes on September 16, 2009, or by calling William C. Walpert at 216-241-2630, ext. 227.
