United Healthcare survey could net prizes for BLET members

CLEVELAND, June 5 — BLET members have until June 9 to participate in a health care survey and possibly win one of several prizes.

United Healthcare is asking BLET members (as well as their family members), covered by the Railroad Employees National Health and Welfare Plan (GA-23000) or the Railroad Employees National Early Retirement Major Medical Benefit Plan (ERMA/GA-46000), to complete a 15-minute Health Assessment online survey. Those who are covered by United Healthcare for medical benefits in the above mentioned plans that register and complete the survey will be entered into a random drawing to win one of several prizes, including a $400 gift card to Dick’s Sporting Goods, a men’s or women’s mountain bike, an exercise bike, or a $50 Visa gift card.

Interested parties should complete the following steps before June 9:

Step 1: Log in to myuhc.com.
If you have not been to myuhc.com before, you must select “Register Now” and enter the requested information to get logged in.

Step 2: Once you are logged in, click on the Health Assessment tab on the right.
If you have never taken the health assessment before, you will be asked to create a one-time username and password. We recommend using the same one you have for myuhc.com.

Step 3: Complete the Health Assessment.
View your health results and take action to learn more about what they mean and how to improve them!

If you have questions about the results of your health assessment, you should call Railroad Health Link at (866) 735-5685.
